Propel Insurance would like to have a custom report written and delivered to them on a monthly basis.
The client is Apollo Inc.
Attached is an example of the information they are requesting to be included in the report.
Please let me know what the time frame will be to build this report and if there will be a cost associated to build this report.

Please also include the column Incident Type (whether the incident is a claim, incident, or near miss)
Is this report to include near misses? I dont see a field that would specify the severity.
The only export that includes near miss data is the work-related basic report. Attached is an example of the work-related basic report. I have listed all fields they want on the report below.
